Personally I use the app Mysugr (Apple and android). It is much more colourful than other diabetes logging apps and the 'diabetes monster' is cute and appealing. And you can export the data to a spreadsheet or PDF document.
I think if you are trying to get every meal, injection, medication dose, exercise for 3 or 6 months (I assume, as those are normal intervals between doctor appointments) onto one sheet you are onto a loser personally, the print will be far too small to read. What is wrong with flicking through several pages, why does it all have to fit on one sheet? Has the doctor told you that, if so what is their reasoning?
It has room for everything (weight, meds, BS, blood pressure, diary for food and activity, graphs for progress, settings, personal information, and settings for reminders).
This is easier than carrying big sheets or a bunch of booklets around....especially to the doctors appointments IMO. It is easy to scroll through and easy to read :)
One touch has a computer program that you can enter in and get reports, my nano has one but it costs over 100.00, figured an ap was much cheaper and it is just a matter of me entering in at night my readings.
so there are plenty of aps, programs that would help you, just a matter of figuring out what you want the doctor to see
