If it is in the morning and your "fasting" was the evening and overnight then you probably have what is called the "dawn effect" You can read about it here:

Many diabetics experience this. Although you did say you recently had surgery and if you are still healing from that, that could also be the reason why if normally you don't have a high after fasting. 140 is not a terrible number, but if it continues you might talk to your doctor about it and figure out a way to get it within normal range.

Have also heard that having protein at night keeps it in decent control.
