I don't think you understand the complexities of self harm! its not "gross" its a coping mechanism!
I haven't self harmed in over 15 years.. what helped me stop was things like exercise.. when I felt the urge I would walk and walk and walk until the urges became less severe... writing/drawing also helped release some of my pent up feelings.. and addressing some of the negative thoughts that come with SI.. epecially things like self hatred and the need for punishment
try something which distracts you... believe me I know how hard it is but it is possible to stop.. and the longer you develop more healthy coping mechanisms the easier it does become

Once you get to the doctor's office you can make your wishes known. That means not notifying your parents; in fact they need your PERMISSION to do so once you are 'of age'. There may be issues like co-pay to resolve, but be honest and assertive with your doctor and is should work out more to your expectations.
