Regarding the house, would it be possible to sell it? Another option might be to make some small changes to help it feel like your home vs. a constant reminder that your parents are gone. Maybe paint a few of the rooms in your favorite colors, and replace a few pieces of furniture with things that you really enjoy.
Taking care of an ailing family member can be a huge challenge. Are there times when your husband could take care of her and the kids so you can go out - even if it's just for an hour? Getting those moments to yourself and being able to recharge your batteries can make a huge difference.
Also, is there another family member or a close friend who would be willing to watch your grandmother and kids so you can go enjoy a good meal with your husband? With so much going on, it can be easy to forget why you fell in love, but taking a little bit of time each week that's just for the two of you can make a world of difference.
If you have the finances, maybe you could also hire a part-time care taker for your grandmother. That might free up some of your time as well.
It might also be a good idea to talk to a therapist. Sometimes, when you're so completely overwhelmed, it can really help to talk with someone who can give you ideas on how to manage your stress in really healthy, positive ways.

As for hubby and the kids, enlist their help. No one is too young to help out. Once you start delegating chores life will get easier. And try to reward everyone once a week for doing their part....whether a few dollars or a pizza night.
Have you laid out possible options for her care? Is there money for her to go to a facility? Is there money for in home care that will give you some 'you time' during every day?
