Depression is a real and debilitating condition that is often misunderstood by family and friends. Its meaning can range from a prolonged period of sadness to an actual mental illness with specific symptoms. Find and share experiences with others who are going through the same struggles.
If you cannot work, .. you cannot work. For those who cannot work, there are certain 'solutions' .. Medicaid for health care and Social Security Disability. These are limited resources .. but probably the only alternatives.
When a person is in a situation where other people are causing problems .. or not helping with solutions, the best thing for that person to do is get away from those people.
From what you say .. your boyfriend should either file charges against family members who are stealing and on drugs, fire the one who is not reliable or get out of the business. Probably getting out of the business would be best.
If he doesn't get up for work, he needs to get up and find another job. If he is suicidal, he needs to get professional help.
On both counts .. you have little to no control over what he does. You have your problems only you can fix, and he has his.
I suggest you go back to your Mother's though it sounds like that is the lesser of two evils right there.
You obviously need to get the heel spur fixed .... probably with surgery. They are not known to 'go away'. As for the post nasal drip .. the only thing you can do is seek another doctor .. who may be able to help you. As for the depression .. probably need to get on a medication .. and that will take a doctor's prescription. You need to find one who will prescribe .. Medicaid or not.
I said it was not easy. From what you describe .. the above is the best advice I have for you. If you do nothing, nothing will happen .. or if something does happen, it will probably get worse. You need to resolve some things .. the load is getting too large. I would begin by unloading boyfriend's problems .. and then start on the others.
