Recently I saw a pulmonologist and she asked me to try the Daliresp again (which I am on in addition to high-dose NAC). I did explain to her the trouble I had with it before. She told me to keep a close eye on my mental state, thinking that as long as I am closely self-monitoring (which obviously I knew enough to hit the brakes last time, no reason for me not to do it again), and believes that it's worth a longer trial to see what benefit I get from it, and if necessary we can increase my Paxil as I still have a lot of room to go up on that. It's worth noting that my primary care does not believe in that strategy at all, would prefer I not take it at all (so now he's suddenly a believer!), but at the very least at the soonest hint of sinking mood dump the Daliresp.
I will say at this time that after a month, I'm not feeling anything more than what seem like appropriate Debby Downer moments as would be expected. I can't say, though, that I'm feeling particularly better, although my understanding is that it isn't so much supposed to improve symptoms, but more to prevent frequent exacerbations/attacks, and it may be too early to judge whether it will do that.
Long story just a little bit longer, if it's the depression that is getting you and still has not lifted (or any other symptom for that matter), do not hesitate to ask a doctor to either increase or add an antidepressant. There is no need to suffer more than we already do so you might as well relieve any symptoms you can. And for those interested in trying it, maybe the key is to have an antidepressant on-board and ready to be increased at the same time you start it.
