My first advice is to remember that we don't control anyone or anything but ourselves. The things your husband is doing are choices he is making for himself and you can not control or stop those. If he is aware that you are unhappy with his habits and behavior, then it's his responsibility now to decide to take action and stop doing them... or not. If his cheating and addictions are hurting you and he knows this, and he doesn't successfully make actual changes for himself, then he might as well be saying "I value these things more than I value our relationship." It doesn't matter how often he says he's going to try again or do better next time. Talk is cheap. He needs to prove it with actions, even small ones.
My other advice is to try to recognize when fear is the only thing motivating your decisions (or lack thereof). Fear's purpose is to stop us from doing things that may hurt us, like jumping off a cliff. But you are already being hurt by your current situation. When you take a calm moment to really dig down, what do you want your life to be like? Do you really want it to be this way forever? If not, then think about what actions you need to take and tell your fear to take a hike. There will be plenty of real obstacles on your path to a better life, but they'll have real solutions, too. Fear of the unknown has no solution, except to face it.

Why is he sweet to you? Because he knows you can't be that stupid to not realize all of this mess, so he's grateful for your willing denial of the obvious.
Your health is at risk with someone who is physically cheating on you.
You mention no CODA groups... how about AlAnon? Same sh!t; different tune.
