My main treatment is massage therapy. Two hours twice a month. I have other issues too so we just work from the neck to hips the whole session. It's a tough place to work out in massage but it does feel better afterward. The relief doesn't last long but its heaven to have it release if just for a little while. By little while 90 min is about the max. I sit on my heating pad as much as possible. I wouldn't know what to do without it.
I'm starting physical therapy in a few weeks to work on strengthening my core muscles to see if that helps any. I'm not optimistic since I have other muscles in constant spasm that have never released for any length of time even after years of massage and trigger point injections but I'm willing to try anything at this point to alleviate some of the pain. I've also done acupuncture, yoga, reflexology you name it and I've probably tried it.
I've been told the opposite of you. That this muscle group doesn't very often go away on it's own. If it does it's after long term intense therapy. Approximately 20% of the population has their sciatic run thru the piriformis instead of under. So says my ortho & neuro anyway.
An SCS has never been offered to me. I'd turn it down if it were. My research just hasn't convinced me that there's enough long term studies to back up its effectiveness. Besides, having something implanted inside my body just gives me the heebie jeebies. I know some people have had success with it but its not an option I'm willing to try.
