Pain Management now days is extremely advanced and pretty compassionate compared to what it was.
As for farmers and old timers Laudanum (Dilauded) has been around for a long time, and whiskey/moonshine and poppy seeds. There are actually quite a few natural plant/herbs that work as well as narcotics, people just aren't as motivated to having to use them.
Word of caution though, if you know you've got a Urine Drop in the near future, lay off the poppy seeds! LOL
But yes-modern day meds and they are still so far behind,,,that is why a lot of other countries are ahead of us when it comes to the medical field
