Maybe the doctor can do another shot, for the pain. Just from my experience is that doctors do not like to keep on prescribing pain meds especially if it is alot, and i am concerned for you what happens when he says no more. Hopefully by then you can have a back up plan that is already working, what i am trying to say is get your refills but try and find something else that will ease the pain, that way you will have enough in back up for when it is really bad, and that they will work.
You know just like me, after taking them for awhile your tolerance will be up there so you will eventually need more then the 5 to get through the day, and you cant really take anymore safety. so if you can find something else then when you do take them they will work with only a few

wow you are so young, i am sorry that this pain is controling your life, i wish you only had to worry about was moving.

There is a progesterone pill which is used in cancer treatment and other gynecological problems called Megestrol. My friend was having periods that wouldn't stop, they put her on this as well as Naproxen. You might want to ask about it.
If your doc makes you feel that you cannot bring these issues to him (them) then you need to either state to them directly that you do not feel comfortable talking to them about your needs (a catch 22 if you don;t feel comfortable talking to them) or change docs.
I would hate to see you caught as above and I know how hard it can be to chnage docs but something isn't working right between you and them if you can't approach them with one of the most important issues for you.
