i'm sure that having all of your medical history current and available would be great.
any paper documentation of what was actually said, even after the fact with approximate dates seems like it could be helpful... such as the "are you ever getting well" comment (i can't remember it verbatim) would be invaluable.
to prove that he discriminated against you due to your illness you should hand write every comment he made, way he made you feel, shifts he took from you etc, etc..all on paper in chronological order.
you might also want to add a list of anything you did for the company that was above or beyond your expected duties, showed strong work ethic or proved your willingness to work.
these are just ideas... i am not familiar with the form you have and i am no way an expert on any of this. i'm just taking some important points from other, non-related, legal issues i have been involved in. take it or leave it, mostly i just wanted to wish you good luck, because i believe what happened to you was completely unfair.
I am doing much better today as I started receiving phone calls from some of my ex co-workers that I had considered friends, now I know they feel the same way about me. Makes a world of difference. AND I heard through the grape-vine that the one that fired me "feels really bad". I am very stoked about that!! Not that I want to put myself back into the lion's den again just that he feels a little of what I felt. I know that's bad but I believe in what comes around goes around and it's starting to happen already!!
