Caring for aging parents can be a difficul time as we become the caretakers for our parents. I would call your brother and tell him to take her to her doctor's office. There's absolutely no reason for you to drive 3-4 hours out and back for these types of things.
Don't let her keep stressing you out, just keep saying NO. This is the hard part, where you have to stick to your guns and keep saying no... I'm telling you, if we don't say no, they'll suck the life force right out of us.
And don't feel guilty. Easier said than done but if you let guilt take over, then she'll have you right where she wants you. It's not easy, I know, trust me.... you're not alone.
It's if the doctor has her admitted into the hospital that she wants me there for. But I don't want to go. Brother can go with her to be admieed to hospital. If it becomes nessesary.
