Caring for aging parents can be a difficul time as we become the caretakers for our parents. If so, then it may be time to get social services involved. From what you have written here and elsewhere, it sounds like your brother's living there is a danger to her health. Part of social services could be an analysis that will determine if brother should be allowed to live with her or not.
Situations are driving this situation to a point where you will have to make some hard decisions. How is your support system in real life? We are here for you on the forum, but that has a lot of limits. Friends and other family in real life are going to be key for you.
