I'm sure your faith has diminished from previous let downs but I would give him the chance. Set a time for yourself that if he doesn't try then you'll need to depart ways. I think if he sees that you are serious; he'll do something.
Perhaps think of things this way: If you stay in this relationship, you will continue to live this on a daily basis forever; if you choose to let go, you have your whole life ahead of you to live as you wish.
As selfish as this may seem, you need to care for YOU first- after all, you is all you have! I also understand it is hard to let go of "familiar" and step forward into the unknown. I did just that and am grateful to myself that I cared enough for myself to take the first step into a new & better life.
I also recognize that every circumstance- every relationship's dynamics are different.
