In the UK to get around the possible kidney issues they do a kidney function test before you go on lithium. Then you go on a tiny dose and they check again after something like a week, then they increase it and take another blood sample. I think I ended up having like 6-7 blood samples while I went up on the dose. Then I think there were regular blood tests to monitor.
The thing about lithium is that it's generally regarded that it doesn't work unless you hit a magic figure of concentration in the blood, below that and it doesn't work, above it and it can become toxic.
