have you talked to your doctor at all about this anxiety? it may really help you to talk to a therapist just so they can work out exactly what is causing the anxiety. in the meantime you could try some breathing exercises, just to calm you down. breath in for 3 hold for 3 and breath out for 3. i know these anxious feelings are horrible, but you can get better!!.
i think it might be a good idea to post this question again in the advice section under the heading ANXIETY AND GOLF/SPORT so people who have had similar experiences can see it, and tell you their experiences.

I could say you've got to get the muscle memory down, I could say a lot of things . But you know you've got to find a way to get it out of your head. Good luck.
