I am sure this is again something that you have been told by Counsellors, but school is temporary. Its a place where we all start to discover ourselves, and the insecurities are natural. The stresses, pressures and anxieties we put ourselves under, again all natural. Unfortunately, bullying is something that is all to common in society now and so many of us have been subject to it. But at the same time, why does someone have to be the bully? Usually because of insecurities themselves. It isn't something you should take on board, or believe. It has taken me years to work through the bullying and believe in myself. We can all do it, but you need to learn to accept and love yourself.
You are perfect how you are, being you. You don't need to compare yourself to others, we all have talents, traits and flaws its what makes us all human and unique. What I consider my flaw, my friends and family joke and love about me.
Its all a learning process, and not something that is a quick fix. Explore some hobbies, make mistakes and learn from them, enjoy meeting new people and discover yourself. You may be surprised by how likeable you are :)
You are doing well, what with everything you are facing in life right now. Anxiety is our body's way of letting us know that something needs to change, much like anger is an indicator that warns us something is wrong. You are certainly not alone in this feeling, and things can change for the better. Its REALLY possible. Since you already know what your triggers are for the anxiety, you can start to address each one in time. For example: thinking about the future, and being a perfectionist. Perfectionists want control over things to make them feel secure. And knowing that we can't control everything in the world makes a perfectionist anxious. That anxiety can trigger a fear response, that leads to an adrenaline rush to either execute their best work, or their worst.
Maybe something positive you can do to break this fear cycle about the future is to stop any negative self talk about the future, and focus on today. Remind yourself about what you can do, and let go of anything you can't control about the future.
We all need to remember that we are human, and that "life happens." So, knowing that we can't control everything can free us to just live life today the best that we can, accepting that we are doing the best we can with what we've got...and letting the rest go. (I am a recovering perfectionist, FYI.)
You are doing really well already, with naming the issues on your heart. So, maybe now, take that next step out of the hell of over thinking and being anxiety ridden. You can do this! I believe in you friend!

I mean - academically it's really good for us. We try a whole lot harder than others. But the wheels can fall off it. Like, not handing in the work because the anxiety made us feel it was worth nothing when in fact it was worth an A.
I want to know if you've been tested for the MTHFR gene mutation which interferes with Vitamin B absorption and puts sufferers into panic attacks a lot. I would start there.
As to the study. You have to force yourself. Anxiety builds as you fail to do the work: anxiety does release as you start to do the work.
Getting through the issues helps with panic. just like folding your laundry helps with panic. Tidying and doing chores and getting things behind you that were in front of you reduces the panic. And you can be freaked out just as easily doing dishes as sitting on a chair.
So find things you can actively do to help soothe yourself. See your doctor, and get a plan in place to help with the anxiety. See the school office and dob in the bullies, too.
