In addition to the link provided by DFS I would suggest having a spouse, or someone close to you, also take the survey but answering the questions in regards to your symptoms/behaviors.
That person can be more objective and possibility insightful. Before I was diagnosed I took the ADD survey on WebMD and then had my wife take it -- it validated the results I received but also gave me an idea of the seriousness of my symptoms. While my result was high, hers was higher. Her test also helped me understand symptom areas that I was not aware of.
