it might be a good idea to condition him to like reading and writing. one way to do it is to get easy-level fun reading and have him learn to like it.
i feel like add/adhd should be renamed "i hate school disorder"... and these so-called "symptoms", ie messy penmanship, could just as easily be symptoms of "i hate school disorder"... if the child doesn't like school, why the heck should he bother to write neatly?!
1. Get yrou child an IEP through the school system. He may not like to read and write because he has a learning disability. It IS possible for a child to be gifted in math and science and totally TANK in reading. The opposite is true for me. I was TERRIBLE with math and science growing up and LOVED reading, writing and history.

Also, young me especially havbe a hard time self-regulating. If your nephew isn't active,try to find him a Kung Fu class and Tai Chi. These specifically are great for teaching self-discpline, meditation, and self-control. King Fu and Tai Chi are related, and both involve deep breathing, etc. Young boys need this. Trust me. I work with pre-K kids all day.
