SCORING:
Questions 1-12 A=0, B=5, C=10, D=15
Questions 13-19 A=15, B=10, C=5, D=0
Questions 20-27 A=0, B=5, C=10, D=15
IF YOU SCORED:
over 275 points: This surgery may be the only way to control your weight, but the life-altering ramifications should be considered carefully.  This surgery will affect your eating style and drastically limit your food choices for the rest of your life.  It will not change any psychological issues you have, but it may change the way you handle these issues when you can no longer use food for comfort, stuff, or drug your mood.
225-275 points: The risks and problems associated with this surgery may outweigh the benefits at this point in your life.  Positive lifestyle changes, such as learning a new skill, starting a competitive sport, or starting a "Gratitude Journal" (featured on the television program Oprah) will be beneficial to you.  Consider this surgery only if health-related issues are making your life miserable.
150-225 points:  You are generally healthy, and you have a good attitude about yourself and about life in general.  Try another serious exercise program and make some small changes in your diet before choosing to undergo any of these drastic surgeries.  Major life-threatening health issues should be the only reason for opting to have one of these procedures.
Under 150 points: You are generally happy with yourself as you are now.  You have a healthy relationship with food and are ordinarily pleased with your appearance and ability to handle life's situations.  Weight loss surgery is not for you at this time.  Your relationships with your family and society in general are good and you do not have major health concerns.
NOTE:
If you answered true for number 19, you should consider that your feelings about yourself are more society induced, and society's views should not cause you to feel a certain way about yourself.
If you answered true to numbers 21,22,or 23 you should know that having this surgery will most likely exacerbate your impulse control problems.  Food has been a coping mechanism that has sometimes worked in the past.  Without food to turn to, your compulsive traits may surface in a way that is more harmful than overeating.  If you seriously consider this surgery, participate in ongoing psychological counseling at the same time.

my score was 280. Wow that was the first time when doing a self-scored quiz that I was totally honest with myself and really I thought I would land in one of the lower categories. But this just shows me that I really need this surgery. When I see the surgeon I am going to print out all my journal entries for him to see how I have been preparing for this surgery.
