The Use of Hyaluronic Acid in Non-surgical Rhinoplasty

Building patient trust in AI‐supported aesthetic therapy relies on transparency about the contribution of algorithms to decision‐making. Patients are generally unaware that AI is being utilized in their therapy or may perceive it as impersonal or intrusive [51]. Clinicians must clearly define the purpose, limitations, and risks of AI algorithms—for example, that a virtual simulation constitutes a probability, not a guarantee. PMMA (Bellafill, Suneva) is a permanent nonreversible filler that consists of PMMA spheres suspended in bovine collagen. Unlike CaHA, PMMA microspheres are not 玻尿酸鼻基底 metabolized or eliminated by the body. The PMMA microspheres remain permanently in the tissue and stimulate neocollagenesis and fibroplasia, with results lasting 5 years or more.13 In 2013, Rivkin14 conducted a prospective study assessing safety and efficacy of NSR using PMMA.

In Group 1 (“Filler + BTX”), subjects received BTX on the first treatment day and in Group 3 (“Threads + Filler + BTX”), 2 weeks after threads administration. Threads were inserted from the nasal tip through a puncture site made by an 18 G needle 2 mm below the nasal tip. Each thread was inserted and covered the length twice between the nasion superiorly and the nasal crest inferiorly (Figure 1). It comprised five P (LA/CL) multidirectional barbed threads (USP 2/0, EP 3), each 120 mm in length.

It provides a valuable minimally invasive alternative to traditional surgical rhinoplasty and conventional HA injection for this patient population. Although the International Society of Aesthetic Plastic Surgery suggested that the nose is an area that is more amenable to surgical procedures, the introduction of advanced dermal filler techniques may favour non-surgical rhinoplasty (NSR) [3].

Startups and established technology firms such as Canfield Scientific and ModiFace, which is owned by L'Oréal, are working with physicians to develop platforms that read aging patterns or forecast surgical outcomes [20]. Adoption varies globally, with technologically advanced regions such as South Korea and the United States leading AI‐assisted robotic surgeries and tele‐aesthetics, while regulatory hurdles in the European Union and elsewhere restrain adoption. Despite challenges, the trend is toward AI as an indispensable tool, reshaping patient and clinical expectations in aesthetic medicine (Figure 1) [21]. The integration of AI in aesthetic medicine presents a paradigm shift from traditional approaches to data‐driven, personalized interventions.
However, it should be noted that the reliability of aspiration remains controversial, as false‐negative aspiration results can occur. Consequently, aspiration should not be relied upon as the sole preventive measure; instead, a combination of multiple safety strategies is advisable [2]. In our study, a small proportion of patients (19.5%) were injected using cannulas, which may reduce the risk of vascular injury compared to traditional needle injections. Despite this, the most severe complication was soft tissue necrosis, often resulting from direct vessel injury or venous compression in the subcutis due to excessive filler placement. HSE, an enzyme that breaks down HA, is commonly used as a treatment for these adverse events [19, 20].
