MRI Scan of the head Clinical History  The history of bilateral lower limb pain and weakness with fatigue and episodes of vertigo along with pulsating headaches, difficulty pronouncing words and blurred vision is noted.  Technique  Multiplanar images of the head were obtained using spin-echo, inversion recovery and gradient-echo techniques.  Findings  No hydrocephalis, mass effect nor midline shift. No focal abnormal signal alteration intracranially.  Posterior fossa structures appear normal and symmetric. Normal pituitary fossa and craniocervical junction.  A rounded signal void measuring about 9mm in diameter is present arising in the region of the supraclinoid internal carotid artery on the right and projecting anterosuperiorly. The appearance is compatable with a berry aneurysm. A second similar though smaller rounded flow void is present arising in the region of the left supraclinoid internal carotid artery again projecting anterosuperiorly, also consistant with a berry aneurysm.  Marked mucosal thickening is present involving both maxillary antra, the sphenoid sinus, the frontal sinus and multiple ethmoid air cells.  Opinion  Rounded berry aneurysms are present arising in the region of the supraclinoid internal carotid artery bilaterally, a larger aneurysm measuring about 9mm present on the right with a smaller mirror-image aneurysm measuring about 5mm present on the left. Appropriate clinical follow up including a neurosurgical consult  with angiographic assessment is recommended.  Features of pansinusitis.
