Coming off Zoloft, going to possibly try lexapro

Ok, please help. I have a history of really bad anxiety, OCD, and depression. I have been coming off of zoloft the past few months. I was on 150 mg for ten years and im slowly weaning down. Im currently on 50 mg and the anxiety is through the roof. My OCD is out of control and i think im dying with a new health issue every day(hypochondria from the ocd) Has anyone ever been on zoloft and had it stop working after a while and then try lexapro? Any advice would be amazing. Thankyou!
