Hi Tozi, here are some comments: as you know, your IGF1 and Prolactin is high. I understand from your other message that you have sleep apnea but no other symptoms that bother you?

Since high levels of GH can cause problems over time, like enlarged heart, and various other things, plus more symptoms developing over time, it needs to be treated. The way you choose to treat it, is up to you. You can do surgery, and if they get it all out, with your size tumor you have good chance of cure and then you don\'t need to take medications or do anything else, unless it comes back in the future. If you don\'t want to take the risks of surgery, you can take medications instead for the rest of your life, assuming they work for you and bring your numbers down to normal.
The issue with the medication is they have side effects, which for some people they could not tolerate, and also can cause serious issues such as gallbladder attacks requiring removal of gallbladder, etc. So, you need to do your research on the meds to make sure you are comfortable with the risks. Otherwise, this is totallly your choice, some people choose not to do surgery.

Many people are treated with Sandostatin/medications for a while before their surgery - this can make your surgery go easier/better. It is not required but it is done sometimes. In a size tumor like yours, this is usually not longer than 3 months. If the medication is reducing your IGF and Prolactin to normal levels, and you don\'t have side effects, then it\'s up to you when, and if, you want surgery.

The sleep apnea can be treated with the CPAP? machine or other things, but if it\'s severe, then your tongue and surrounding areas are probably enlarged, which means you must get your Acro under control.

if you decide on surgery, the med research studies say that results are related to the experience of the surgeon, so you should get the most experienced pituitary surgeon you can (ideally several thousand surgeries). If you don\'t have a surgeon like this in Turkey, you should consider traveling elsewhere, if that\'s possible. There are top surgeons in USA and Europe that I can give you some names, let me know if you want this.

I hope this helps you. let me know if you have questions. don\'t let the doctors decide for you... you must decide this important thing yourself, after gathering all the facts (do your research!). all the best, Sunny
