I was on 200 every other day and yes the patches came off. Tegraderm bandages helped some. They're the plastic they use on IV lines. They're very expensive in the stores, but the Dr's can get you a box.
As you know, the only thing that works for me is the pump and I have dilaudid in that with percocet for breakthrough medications. I hope this information helps you.

I was on the 100 patches and used silky tape around the edges The sites were interchangeable due to blisters. Unfortunately I developed a reaction to the medication.

Sometimes, when I'm running short, due to having to replace a loose one, I cut them in half, to make more patches...although only 25mg. That doesn't help the pain, but for me, it does help prevent withdrawals. But....DON'T cut them if they are the gel kind! I have the matrix type patch and my Dr's nurse told me it was ok to cut them. Your pharmacist can probably advise you on this.
Yeah, I totally get what your issue would be, if you are on that kind of dosage. Seems like a breakthrough med would help a bit.
