Polymyositis (PM) is a type of chronic inflammation of the muscles (inflammatory myopathy) related to dermatomyositis and inclusion body myositis. The inflammation is predominantly of the endomysium in polymyositis, whereas dermatomyositis is characterized by primarily perimysial inflammation.
Plaquenil is a rash reducer but not a rash remover. The steroid creams are known to help with hand rash. I would not know about getting off the Plaquenil. Most people who completely get off their steroids usually stay on the other DMARD. I would love to get off of Cellcept, but that just is not going to happen.

There are different medication dosage levels for each person depending on disease activity. Tapering is not a race. Doctors will use different tapering schedules. My taper was very slow and down by a half milligram every fourteen days. If I did not totally feel well and stable, then I would stay on a particular level of Prednisone for a while until I felt stable.
It is not uncommon to flare while tapering, so try and be patient. I learned a lot about patience from DM, and yes, patience is not always easy. Do not be afraid to ask friends & family for help or assistance during this difficult beginning period too. I had to ask for help shopping for groceries (the bags were too heavy!), I needed help getting dressed, driving, etc.. at the beginning of my DM.
Things will get better, but they do take time and patience.

What is your dose of Methotrexate? How long was your taper down to 15 mg of Prednisone and what was the schedule? I found an every other day schedule was best for me but understand that is not a regular schedule. I stayed at some levels for 3 months or more. I was on 3 mg for over 6 months. I went down 1/2 mg each taper once I got down to a lower dose.
One discovery which has made a major change for me is the past year of acupuncture treatments which have restored muscle strength and balance.
