Polymyositis & Dermatomyositis Support Group
Polymyositis (PM) is a type of chronic inflammation of the muscles (inflammatory myopathy) related to dermatomyositis and inclusion body myositis. The inflammation is predominantly of the endomysium in polymyositis, whereas dermatomyositis is characterized by primarily perimysial inflammation.
I have DM. In my third year with DM, I developed overlapping scleroderma. Scleroderma is a common DM overlap which may also involve skin issues. You may want to check out Sclero.org and see if symptoms look familiar to you on that site.
The Myositis Association website is a good place to learn more information about DM. I think the requirements for a myositis diagnosis have become more comprehensive.

A skin biopsy will allow them to do pathology on your rash to help further diagnose. Since I have PM and no rash, I'm not sure that process and if medications impact the result. My rookie thought is that it would not because the rash is present.
On a side note, I would question the IVIG for DM if you still have a rash. To me, that would indicate it isn't working the way we needed.
You need a great medical dermatologist. The DM team at Stanford is great, i go there, but John hopkins and Mayo are also great.

As for which type of doctor, some of us have rheumys and some have neuros. But you might want to start with a derma who will take skin biopsies, as that can often diagnose DM. Then if the skin biopsies show DM, you can decide on a rheumy or neuro at that time. Best wishes to you.
