Polymyositis & Dermatomyositis Support Group
Polymyositis (PM) is a type of chronic inflammation of the muscles (inflammatory myopathy) related to dermatomyositis and inclusion body myositis. The inflammation is predominantly of the endomysium in polymyositis, whereas dermatomyositis is characterized by primarily perimysial inflammation.
I'm not a doctor, but here's what my neuro explained to me. He said that with PM, the muscles get so inflammed that they can't receive messages from the brains and nerves (thus we gradually are paralyzed). While the muscles are no longer responding, they begin to atrophy. He told me that the prednisone is a very strong anti-inflammatory. The purpose is to reduce inflammation so the muscles are once again capable of receiving signals to move. As the inflammation subsides, we conceivably should be able to reduce the dosage. Prednisone also suppresses the immune system. Hope this helps.
BTW, I am off all prednisone since May and holding my own. I'll never be what I was before PM, but I'm not as bad as I was when diagnosed. Also, my one doctor recommended a supplement called Lauricidin for my chronic problem with Epstein Barr Virus. She felt that the EBV could have triggered the PM. The best part of this supplement was that it greatly reduced my muscle and joint pain--a very welcomed side benefit. Best wishes to you.
I would talk to your doctor about your concerns. I'm sure he has a plan.
This is a rare disease and there really isn't a treatment protocol for everyone as each one is unique. If your pred is reduced they introduce another drug to take over for the pred - I am on cellcept. It is a constant gauging of blood work and how you feel to get the right balance.
With the pain you are experiencing - you may be tapering the pred too fast. What I did was have my blood work checked while tapering to see if it was a flare or if it was steroid withdrawal,
I don't think we get healed - it varies some go into remission with meds or without meds.
