Disappointed after 1st egg retrieval
Hi everyone,
I'm new here and new to IVF. I started my first IVF cycle in March and there have been many ups and downs already. I've been reading everyone's posts and they are very helpful and encouraging. Thank you!
I was very positive going into the whole process since my RE said I have good ovarian reserve and all of my previous ultrasounds showed a good number of follicles/ovary. It took about 6 months to finally get started on my first IVF cycle because I'm doing PGD and we had to get the genetic test in place before starting. So, after months of waiting, I was anxious and excited to get started. At my baseline visit, I was surprised that the ultrasound showed only about 6 follicles on each ovary. My RE also seemed slightly surprised, but I think she didn't want me to be worried about it. I started the stim meds, but the follicles were growing so slowly. Eventually, my doctor increased the doses of my meds. My hormone levels were going up as expected, but the follicles just weren't growing. I started wondering if it was even worth it to do the egg retrieval procedure this time around. But, my RE encouraged me that there were some smaller follicles that could possibly still work. She mentioned that sometimes there are follicles that contain mature eggs, but they just don't grow in size for whatever reason. She said we wouldn't know until we did the retrieval. So, I tried to stay positive. We decided to go forward with the egg retrieval at the point that we had four larger follicles and lots of smaller ones.
We did my first egg retrieval two days ago. Everything went well and I'm recovering well. I was so happy after the procedure when I heard that we ended up getting 13 eggs! But, then the next day, my doctor called to tell me that they were concerned with the quality of the eggs and that only 4 of the 13 had fertilized. This was such disappointing news. Now, I just need to wait for a few more days to see how many survive to day 5. I'm trying to stay positive, but I feel like it's a rollercoaster-- good news, then bad news, then waiting, then it starts all over again.
I expected this process to be stressful, but it's been more emotionally draining than I imagined. On top of that, my husband just started a new job and has been exhausted when he gets home. I know he is trying to be supportive, but I can tell that he needs a mental break too and doesn't want to talk about IVF over dinner. I'm trying not to overwhelm him, but to share the most important points and how I feel each day without spending too long on the topic. Reading everyone's posts on here have helped give me moral support as well.
Similarly, for my age (36), the doc continues to suggest I have a good follicle count (~14) and AMH level (3.5). With my cycle day 9 US on Thursday, I am excited and anxious to see where we stand... But trying to temper my expectations, and your journey illustrates the very bumpy ride.
Did the number of follicles significantly change from other ultrasounds? Do you feel like PGD has been a positive contribution to the process? Are you doing a fresh transfer, or freeze-all protocol? How many embryos do you intend to transfer?
Yes, for some reason, my number of follicles dropped significantly from other ultrasounds done about 6 months ago. At that time I was 34 and my AMH was 4.68. Since then, I've turned 35. I asked my doctor if this drop was normal and she said no, not unless a patient had undergone something significant like chemo or radiation, which was not the case. Of course, numbers can vary from cycle to cycle, but not by that much! So, I'm not sure what happened this cycle. I'm hoping it was just a one-time thing and if I need to do another round of IVF that my follicle counts will go back to what they were. But, for now, I'm just focusing on this cycle and taking it one step at a time.
We won't know the results of PGS/PGD for a couple more weeks, but I'll keep you posted on how that turns out. For us, the genetic testing is the reason we're doing IVF. I'm the carrier of a genetic disorder, I want to be sure that it's a healthy embryo before transferring.
Due to the time needed for the genetic testing, we're doing a freeze-all protocol. Unfortunately, during the stim meds, I also developed a polyp in my uterus, so that will need to be removed before I can move forward with the transfer. We haven't decided how many embryos to transfer yet. I'd prefer it to be just one, but we'll see when we get to that point.

To avoid duplicating, I just recently posted more about my background (Subject: Cycle Day 9 here I come!). But in short: this is my first IVF cycle, we are here (as far as we know) due to male factor infertility, and will use ICSI. We are also doing PGS, and our clinic recommends FET.
Meds are OK - I anticipated my emotions to be higher, but I feel more physical discomfort in my back, and now my belly. My diet and exercise has remained relatively the same, though perhaps less carbs and sugar, and a more diligent effort to walk (thx, fitbit!). And my mantra: Strength in my heart (breath-in), clarity in my mind (breath out)... now added-in: One day at a time - given all of the unknowns and waiting of IVF, seems right ;)
Sounds like we have a similar plan-- ICSI, PGS, FET. But, as you said, one day at a time!
Three of our embryos made it to the blastocyst stage and can be biopsied for genetic testing! One is good quality and the other two are fair. I'm happy with these results for my first cycle. Thankful that we got something!
I've decided to do a second round of stim meds/egg retrieval so that I hopefully get a few more embryos to work with. Then, we'll batch all the embryos from the two cycles together and send them to the genetic lab for testing (PGS & PGD). Crossing my fingers that this cycle will be even better! I'm feeling positive about it today and I know it will work out eventually.
We're trying to think ahead as well-- My husband and I would love to have two children, if possible. Since I'm already 35, it's probably smart to get as many eggs as we can now. Who knows what may happen in a couple more years with my egg quality. It will cause more of a delay now, but in the long run, I think it will be worth it. Has anyone else done this? How many embryos did you have left for a second pregnancy?
I am now heading into similar territory after my egg retrieval yesterday. They were able to retrieve 22 mature eggs (!!), but I learned today that the quality is overall poor. There are 20 in culture, with 15 showing signs of normal fertilization (we did ICSI), but given the quality, the embryologist believes only three are likely to make it to blastocyst. They'll call again on Thursday with a clearer picture, and on Saturday with final results. We plan on PGS, and FET protocol.
So while there is something, I am already contemplating next steps (ugh, what about my mantra?!) - was it the cohort of follicles, or my ovarian physiology?would we do another cycle? what will insurance help with? I am 36, and feeling the clock... (One day at a time! It just takes one!)
