I talked with the sperm bank here that we would need to use. It would be $775 for the initial visit, plus lab work is needed. Then, they would give half that sample to the IVF lab and if it was usable then they would freeze the half they kept. If it was no good then we would need to get them another sample and that would be an additional $355. And it would be an additional $355 for every sample going forward. Sperm freezing is covered for a year and then it would be another $600 for them to keep it longer.
