I am about to have to give up the war against my body and ask for stronger meds. I take Ultram 50mg every 6hr (I have headaches if I take more), Neurontin 900mg 3x a day, Tylenol 1000mg every 4hrs, and Motrin 800mg every 6hrs, none of this is even giving me relief this week, I have a mass/tumor/cyst (no doctor gives it the same name) it is so small and I am so messed up from scar tissue and Endo that no surgeon will do anything to get it out or even biopsy it. I have been at a high level of pain today and yesterday, its to much for anyone to handle.
Now what long acting pain meds are out there, I am a nurse but pain meds as medication are not my specialty, I was a drug and alcohol testing nurse as well as minor care. Is there anything that you can take once a day or once a week or just every 12hrs I get so tired of shoving meds every 4 hour and my poor liver must think I have gone crazy. I put patches on my mom but I think they were for cancer patients every 3 days but she had to take MS Contin for breakthrough pain, she was high a lot but I think her pain was under control.
Any advise would be appreciated.

I'm trying to exercise daily. I was doing fairly well until I sprained my ankle 2 weeks ago but now I'm getting back on the horse. My first thought after reading the first paragraph is that your liver is not going to stand up to that abuse for any amount of time (Yep, I'm a former nurse too). For that reason, if nothing else, you need alternate pain meds.
The patches you talk about giving your mother would be a great way for you to go. I use them and they work great for me. They are called Fentanyl or Duragesic patches and they come in many different strengths, so the meds can be attuned to you. Just enough but not too much. They are often used with a 6 hour breakthrough pain med, but if you have the dosage right on the patch, you need very little for breakthrough.

I guess I should say that I have chronic pain, fibromyalgia, rheumatoid arthritis, and rheumatoid vasculitis, and a whole bunch of garden variety disorders. I have found that doctors are more likely to give out meds that are non-narcotic, than the narcotic meds.

Also just wondering, Notthosboss, you said your doc did not want you on narcotics and then he put you on methadone? Methadone is certainly a narcotic, I dont know if he informed you, but it is actually a very strong one at that. It is less addictive than other opioids though and that is probably why your doctor prescribed it.

But again, talk to your doc. Only the two of you can figure out what is best for you. You should probably even ask to be sent to pain management where they know in more detail what is available and how best to work with you and your sensitivities.
