Welcome to the group. The initial diagnosis hurts so much and it take a lot of time to wrap your head around it. You find over time you cannot do everything and learn to accept it. You do the best you can and your efforts will be appreciated by grandma. She understands, but she may lash out, which is quite normal.
Take one day at a time and don't get ahead of thinking.

I am a sahm trying to lose her pregnancy weight in my mid 30s. With little time and energy to spare this is an uphill battle.
To lose pregnancy weight I've gained with both of my sons. I have 20lbs to lose and it is hard!!!!
