Immunizations II: The First Year

Continuing my discussion of childhood immunizations (AKA vaccines), I'd like to now share those which are recommended during the first year of your child's life.  Along the way, I'll also discuss when the these vaccines are specifically recommended by the Centers for Disease Control and Prevention (CDC) and American Academy of Pediatrics (AAP)...not so much to further any vaccine debate but just to follow a general premise I have which is a little knowledge is a good thing.

                                                 

So in no particular order, here are the diseases that make up the six recommended vaccines during year one:

 

Diphtheria, Tetanus and Pertussis (DTaP): Recommended at 2, 4 and 6 months of age, as well as boosters at 15-18 months of age and 4-6 years of age, this vaccine includes 3 different components.  Diphtheria manifests as a membranous coat in the tonsillar and pharynx region.  Although a mild form of the disease may appear as a simple upper respiratory like infection (cold) accompanied by fever, more severe complications include obstruction of the airway, paralysis, cardiac complications, and even death with a 5-10% case mortality rate.  In the U.S., actual cases have dramatically decreased, but worldwide, outbreaks continue to occur, particularly in the former Soviet Union.  Tetanus, yes, the disease we think about when we cut ourselves on a chain-linked fence, can appear in a generalized form: with severe muscular spasms and even a locking of the jaw with a case mortality rate of 10%, a localized form: spasms around the site of infection, and a cephalic form: where cranial nerve involvement occurs following wounds to the head and neck.  Specifically, it is a neurotoxin produced by the Clostridium tetani that causes the disease.  The very contagious pertussis (AKA whooping cough) may appear as a simple cold but in infants, can cause apnea, seizures, and death.  Based on data from in the 1990s, the case mortality rate is 1% for children less than 2 months of age and <0.5% in infants 2 months to 1 year...which serves as a good reminder for adults to get their booster as earlier in this decade we were finding adults (fully immunized as children) were acquiring the disease secondary to waning immunity and then infecting infants not completely immunized.  And for those interested in what the "a" represents in the DTaP...it stands for acellular.  It was designed secondary to some concerning adverse reactions to the original DTP vaccine.

 

Polio: Spread via person-to-person contact via infected stools or oral/nasal secretions, polio was so very prevalent before the first vaccine was produced over 50 years ago (please see my Immunization I discussion for a bit more on the polio vaccine history).  Although many infected persons never have any visible symptoms, approximately 1% develop paralysis often leading to permanent disability and occasional death.  These days, there are two vaccines available, one being an oral form and the other being an inactivated form which is injected.  Doses in the first year of life occur at 2, 4, and a 3rd booster between 6-18 months of age and a final booster between 4-6 years of age.

 

Haemophilus influenzae b (Hib): Infections caused by Hib include meningitis, pneumonia, and epiglottitis (a very very bad throat illness).  This vaccine has been remarkable in preventing the above infections (where invasive disease has been decreased by 99%) and is recommended at 2, 4, and 6 months with a booster in the 2nd year of life.  Of note, depending on whether it is part of a combo vaccine, the vaccine schedule may change.  Also, in the U.S. we are currently dealing with a shortage of the vaccine so a total of 3 doses are all that is recommended at this time.

 

Hepatitis B: This disease attacks the liver and besides leading to lifelong infection, it can cause cirrhosis or scarring of the liver, liver cancer, liver failure, and death.  In infants, the primary concern is perinatal transmission...not so much from in utero exposure (pregnant mom to developing fetus), but during the time of delivery.  This is why pregnant women are tested for Hepatitis B status during pregnancy.  3 doses are recommended with the first being shortly after birth.

 

Pneumococcal Infections: Similar to the vaccine schedule used for Hib, this vaccine (the 7 serotype form...i.e., covering for 7 different types of the Streptococcus pneumoniae bacteria) is used to prevent a variety of childhood illnesses ranging from ear infections to sinus infections to pneumonias to meningitis.  Since 2000, when this vaccine was first used, all invasive disease caused by this bacteria has decreased by 80% for children less than 2 years of age.

 

Rotavirus: Although a different vaccine existed a few years back for prevention of rotavirus...which happens to be the leading cause of gastroenteritis (AKA "stomach bugs") among children worldwide, the two oral vaccines that currently exist are the newest on the block as far as what is available during the first year.  In the 2-3 years since we've been using this vaccine, there has been a tremendous reduction in the number of cases here in the U.S., and therefore, emergency room visits and hospitalizations.  And, to date, no increased incidence of intussusception (a concern of the vaccine from a few years ago)...where a blockage of the intestine occurs following the "telescoping" of one part of the small intestine into another part.  Depending on which of the vaccines your child is given, it is either a 2 or 3 dose vaccine.

 

And, of course, just as a reminder, the yearly influenza vaccinations can begin at 6 months of age...usually in the Fall or Winter.

 

And lastly, I do apologize for the length of this discussion...but since there is so much good information about the vaccine-preventable diseases we have available for our infants, I want to make sure you get as much as possible.

I'm sorry - but that is way too much for their little immune systems the first year IMO or even the first few years! No disrespect intended - you go to school a long time as a Doctor and I know some vac. are warranted, but its the drug companies pushing it more than anything now IMO. I did follow what the Doctors told us to be a good mom 21 and 18 years ago to a point till they got older and I learned more. Luckily mine weren't too affected, but I also know personally many who had kids who were literally different people right after they got their shots for Kindergarten. I have not had mine immunized beyond the basic MMR schedule. I believe in homeopathy and vaccinations are along the same principle. I had to have shots as a reservist, too. But some of our friends were affected by the shots for the Gulf War. I've never gotten flue shots because they give me the flu!! What is the point? I have a cousin with polio. Smallpox is warranted, too. But I had measles, mumps AND rubella as a kid. If nothing else - there are natural things you can do to help the system cope with the onslaught. To me - its just too much all at once..... period. They require a ridiculous amount of shots now. I don't have to be a Dr. to have common sense. In the end we all have to educate ourselves and make the final decisions as far as what we put into or bodies or have put into our children's bodies! The medical community has no right, though, to try and force it either. Morals would be better preventative care than what they have kids do now. My daughter was a virgin when she married at 19 and my younger one is still one at 19. They have learned how to be friends with guys first without intimacy. Kid's are so confused these days.... That's another topic, though. No disrespect, Doctor. I use to work for Doctors as a medical assistant. I just disagree. I've always been more "naturally" inclined. I don't care what the supposed research claims, either, with regards to autism and its literal rise along with vaccinations. (That claiming its not related). I work in Special Ed. I see it every day!!!! SOMETHING IS WRONG.

Dr.J all I know, my 10 year old did not have many vaccines until 18 months and never had Hep A or B until several years old (sorry I need to get the record out to be more precise) . He is typical.

Oli was a stronger baby and had nothing delayed, so I did as the health center suggested at the 12 months, and later realized I had him shot 6 times with 9 innocs. He was never the same. His teacher notices the difference a few weeks later and it continued to get worse and worse until I treated with a chelation therapy. Now he is getting better, but he has a lot to overcome, and the society is hateful and people do not understand the importance and urgency of EC treatment, and the doctors never get a clue, so the issues get worse.

I understand the extreme feelings this topic generates, but I'm disappointed on how things always turn cruel. The point of this blog was just to deliver some general information about some of the vaccines given during the first year...I specifically did not share my particular viewpoint in this blog as I didn't want it to make it the focus. But since this always seems to be the direction of any immunization discussion, I'm going to ask that we all keep this more civilized. In fact, I'd love to hear some alternatives members might have to the vaccine schedule (and I'll be more than happy to respond).

And for those just looking for a bit more information about some of the vaccine preventable diseases, my next blog will discuss mmr, varicella (chicken pox), and hepatitis A.

Ok then HLKS I guess you know it all. When I want to be a mess of a human with an attitude I'll ask you for advice.... Since you know everything about how the symptoms are....

Dr. J I do apologize for the fact that there are kids indeed damaged by vaccines. There are also others who are disrespectful and mean. We are being jacked by the doctors an the educational system and the insurance and the legislature. We are jacked by the mean people who are very sick and uneducated as well. Sorry to exist to mess up your vaccine blog.

HLKS... the side of science or the side of the pharmacutical companies??? Who are making a mere mint off of everyone and inturn hurting kids and at whos expense???? Really, I am just wondering is it a good idea to ingest mercury??? Because regardless of what you think it is still in many of the vaccinations, but if that is what you choose to do I don't begrudge you. The thing is don't begrudge me for not choosing to inject it into my children. To each his own.

Ok, so if I use a new face lotion, and 5 minutes after application, my face turns all red and blotchy, it is safe to assume the two are not related.....? bull pucky.

If after 25 years of smoking a persons lungs are black and full of mucus,,, not related.

If after spending all day in the sun on the beach ones skim begins to turn red and peel.... not related.

If after eating a peanut, a person stops breathing as their throat closes ..... not related

If after being hit by a car one dies..... not related....

I see a fault in your reasoning... "symptoms...often appear around the same time as...not related. correlation is not causation" I can give you more examples of the correlation of symptoms as a direct result of their causation if you like.... millions

symptoms of autism often appear at around the same time as certain vaccines which gives the appearance that they are related. Remember that correlation is not causation. It's not about believing an opinion. There have been studies... they've looked into this at depth. Science just does not support you. It's superstition and fear. Fear of doing something by action rather than inaction. But it all amounts to the same thing. Vaccines are wonderful and have effectively wiped out many diseases from the US. You are entitled to make whatever insults you like, but I'd much rather be on the side of science.

My own son met every single developmental milestone before his booster immunizations. He walked was begining to talk simple words etc. He was a wizard with puzzles and shape sorters etc was very mechanically inclined. Then after his booster shots he developed a fever, became lethargic, we rushed him to the hospital. He lost weight, I have pictures, he was a shadow of his former self. Slowly his words dropped off, he wouldn't respond immediately to his name. He was no longer able to put together the same puzzles and shape shorters. He bagan to tantrum in great frustration at being unable to communicate, he began to bang his head and stim. Became fascinated with lights and spinning toys. I saw this "back slide" and couldn't put the pieces together until we began looking at pictures and video. My sister-in-law's son was also verbal prior to his own back slide or regression into Autism. And while certainly the fact that 2 boys within the same extended family shows a susceptibility, I believe they were the victims of vaccine induced Autism. To further the theory, the boys at that time also shared the same pediatrician. And probably the same vaccine stock. Nothing at this point can convice me that this was not vaccine related.

Gee, I miss the party. FYI HLKS there is always someone to be mean to mean.

What scares me is a mean person jabbing babies and making them sick with autism. I hope you find some peace and stop being a mean fool. The families suffering with vaccine injured members will never believe your debate. You obviously cannot inderstand.

And also HLKS in regards to herd immunity.. Its mainly adults whose artificial immunity to these diseases is waning, immuno-compromised children, and/or children who didn't mount a sufficient immune responce from being vax'd to be "presumed immune" who are "at risk" from a lack of herd immunity. It's mainly adults who would suffer as these diseases are far far less deadly to a child as they are to an adult.

That being said I am not scared of my child getting these diseases as I am not injecting this crud into their chemically sensitive bodies to protect your child. I do not believe I am "hurting" my child by not vaccinating them at this time. Sorry, make sure you get caught up on yoru boosters because the amount of unvaccinated children is rising and will continue to as the truth about the dangers of vaccines gets out. Ignorance is bliss but you can't deny what is in your face anymore.

Is it just me or does intussusception sound much worse than the vpd the shot was designed to prevent. Who knows what else they cause or what vaccine will be replaced with a "better" version.

HLKS I am highly concerned that you have vaccinated your child and you don't even know for which diseases you have done so. Seriously do you not read the handouts they give you? wow. Why don't you google Hannah Poling and again state that it sounds moronic. What is moronic is to think that injecting these known neurotoxins (and other really disgusting chemicals) in mutiple doses into bodies most incapable of processing them wouldn't cause somekind of damage.

Dr Jeremy, I say this with all respect but.. I am still waiting for the good information. This is nothing more than a dumbed down version of the crap the CDC usually pumps out. Why may I asl if perinatal transmission is the primary concern for infants ("thats why all moms are tested during pregnancy") and Hep B is usually a risk for IV drug users, gay men and others who engage in high risk sexual acts we routinely vaccinate ALL infants (knowing there activites are clearly not at high risk) regardless of the mothers Heb B status?


Lastly anyone, does it piss you off that we could sue the $#!t out of the pharm companies if Viagra causes heart issues (or any other drug for that matter), but a tainted batch of any vaccine can leak out into circulation and there is not a single repercussion that can be had???

The only thing that is moronic... may I add is closed minded people. Like I said walk a day in my shoes and see some video I have of my son who was speaking, playing, etc.... then after the vaccinations lost all language and started to stim. DO you know what it is like to have that happen to you??????????????? I don't think you do, so like I said live this then tell me I am a morone. I have done more research on autism than I care to talk about... Like I said HLKS I have read your discussion in the past and there is no talking to you anyways because you are one of those people that are always right. You ask for advice but never take it because you have all the answers anyway. Please, don't call situations moronic when you have no idea as to what you are talking about.

Studies have proven that certain vaccinations (tainted batches likely the cause) were very much attributed to autism. And I'm certain that a mother knows when a child has been affected. We mothers tended to be absolute gatekeepers on our children's health issues and any signs of abnormal behavior that develops in that child. To say that such things don't happen or exist is just uneducated. I hope you never have a child that suffers at the hands of pharmaceutical company mishandling or greed.

All I have to say to the people that say vaccinations do not cause autism is mitochondrial disorder. There are people that have gotten autism from vaccination.... Anna Poling. She won her case in court for have gotten autism from vaccinations. Plus, like I said walk a day in my shoes watch video of my son who was developing fine until he got lots of vaccinations in one day. Easy for you to say when you don't have autism in your house. The problem is that the CDC doens't want to listen to us parents who may have something to say about this. Maybe you should go hlks into the vaccinations group here on DS and look at all the kids that have died due to vaccinations and try telling those moms that their kids didn't die due to complications from vaccinations. I have said it once and I will say it again.... vaccinations need to be cleaned up and spaced apart!!!!! I think you may not be listening to people on here no one is saying stop vaccinating. As far as autism not coming from vaccinations that is not true. Come hang out with some of the moms on the boards in the autism groups. Like I said to you earlier.... you have healthy children know one knows where autism comes from. No one can say 100% that vaccinations don't cause or contribute to autism in children. My doctor won't even tell me that!!!! Must be nice going to bed knowing that you are always right.... well every discussion that i have seen with you in it anyways. Whatever I am done with this.

In March 2007 I received a series of post exposure rabies vaccines. Life as I know it will never be the same. I spent the first 4 months in abject misery with the worst 'hangover-type' symptoms imagineable; these have subsided to a small degree. I also have inflammatory arthritis, memory and co-ordination problems(possibly from an 'unknown calcified mass' that suddenly appeared in my brain), and am in constant pain...the list goes on.

I can no longer marathon run, distance swim, ride, or have a snowball fight with my children...

I am, however, still alive.

The alternative for me would have been death, so I made the only possible choice and was vaccinated. I am grateful that I was ignorant when my children were vaccinated, because I don't know what my decision would have been had I been aware of the dangers.

Parents need to have ALL the available information to be able to make their own, informed choices.

i beg to differ , as a nursery nurse who works with children who have special needs, i have spoken to so many parents who have autistic children and im sure they will agree that there is almost certainy a link .
lets face it.... why do tony blair and the rich and famous go for single spaced out jabs !! got to be something in it !

Have you actually read the studies? What a joke. There are absolutely zero studies that prove that vaccines do not cause autism. Read the studies:

And we are not just talking about autism - we are talking about immune dysfunction - allergies, auto-immune disease, ADHD, asthma. Our children are sick. Why do the countries with the lowest number of mandated vaccines (less than half that of the US) have significantly lower rates of autism and significantly lower rates of infant mortality under the age of 5:
