Is this really PMS or hormonal imbalance?

For the past 6 years or so I've suffered from horrendous symptoms the few days before my periods. If my period doesn't come for some reason, I will have these symptoms until it does come (one time they lasted every day for 2 months). They are:
Dizziness
Nausea
Hot flashes/flushing
Chills
Near Syncope
Weakness and trembling legs
Loss of peripheral vision
Frequent need to use the bathroom
Anxiety (usually because of feeling so sick)

I've researched estrogen dependence and my mom thinks I have it as do I and should try progesterone cream. Has anyone tried this before? My GYN doesn't want to deal with this for some reason - she thinks at 24 my symptoms are all sugar related except my blood sugar is always normal during these episodes. I take BCPs to treat the symptoms but they often lose their effect after 8-10 months.
I already take B-Complex, Vit. C, Vit D, Flax Seed Oil, Multi-vitamin, Kava Kava, and a Mag+Ca+Zinc combo - what more is there to take?!?

I'm so very frustrated and mad. These symptoms caused me to leave work the other day and I felt so defeated by my own body. Does anyone have advice? I'm a nurse but I work with kids - this is so unfamiliar to me and I'm looking for woman-to-woman advice.

" You poor woman !! Your gyn wont deal with it?? I think you should get another gyno! Im 25 and ive also been having gyno issues but the gyn keeps saying that everything seems to be normal. Ive had 2 kids, have you had any kids?? and if so, did your symptoms start after you had your kid/s?? I wish i could offer you advice but im in the same boat. Defenitely dont give up with finding a doc that will do something for you, because thats ridiculous. Hope you feel better soon (((((hugs))))

" No, I've never had any children. I hope to in the future. My gyn has been awesome for me for 10 years - so to think of getting a new one is stressful. But I think I've outgrown her. I know I have some type of insulin resistance or something but it is completely separate from these PMS days of hell. She doesn't seem to think the two are caused by different thing - but when I read the literature on estrogen dominance - I SCREAM the symptoms. "
