Medicare is the name given to a health insurance program administered by the United States government, covering people who are either age 65 and over, or who meet other special criteria.
This is the one line answer but there are exceptions for earlier start. For example if you are receiving SSDI or have certain disabilities such as ALS or end stage renal disease, or if you have earned enough quarters to buy in at 63 by paying a premium over and above what the current 65(+) receiptants pay.
I am 75 and I currently pay $96.40 amonth for part A & B. I earned enough quarters but still have the premium deducted automatically from SS benefits. If you Buy in pre 65 it could be as much as $300 (+/-) a month dependent on the state you live in.

that's what happen with me and my bipolar at 19 it's on going as long as your disabled.
for my mother she got it at age 50 when she was placed on SSDI most of the ppl in apartment complex got it when got on SSDI as well.
