things have moved pretty fast in the last week: diagnosed with severe male factor infertility (0.5mil/ml, 35%mot, 1%morph), met with urologist (no cause found/identified), got LH/FSH/test labs (all normal), and today met with the RE.

Our meeting was very helpful, reassuring, productive and generally all around good. Since our goal has and will always be to use my sperm, ICSI was an obvious choice for us. Everything with my wife has been, and we hope continues to be, normal. At this stage, it's all about the eggs! ICSI eliminates the need for normal motility and morphology, and to a certain extent concentration: all you need is one sperm per egg. We have a few genetic tests we're waiting on, but the chances of them being abnormal and changing our plans is highly unlikely.

We've got out preliminary calendar schedule (all dependent on when day 1 of my wife's cycle starts) and our likely drug regimen (Follistim, Menopur, lupron, progesterone). Now we get to hunt for the best drug prices, but we know it'll be a lot.

The last remaining questions we have to answer are whether to transfer 1 or 2 embryos: so many factors go into this decision like quality of embryos, number of embryos, personal feelings about twins!

I feel for and have so much love for my wife who now has to go through all these drugs and procedures, all thanks to me. And all I have to do now is close the door and fill the sterile cup!

" i would transfer 2.. we transfered 3 and still failed.. but we have both factors.. if the only problem here are the swimmer give it a chance and tranfer 2.. when are you guys expecting her cycle to start??? "

" It all depends on your wife's age and fertility issues (if she has some, or not). If she is below 35, has no major issues and the embryos are a good quality, transfer 1. This derives from personal experience:
I did IVF with ICSI when I was 25. My husband had a low sperm count and bad quality, I had nothing else than a thin endometrium. Our embryos were of a good quality. Our RE recommended transferring 1, but we insisted on 2. We got twins and were thrilled, but didn't listen to all those who told us multiple pregnancies were risky. I went into labor at 6 months and my babies did not survive more than 2 days.
I'm not saying this could happen to you, but I'm just asking you to be cautious. Talk about it thoroughly with your wife and your doctor, and follow medical advice.

" We are in a similar situation. We have a lot of male factors working against us, and nothing on my end that we know of. In late March/early April, we did a fresh IVF cycle all ICSI. We started with 5 of 7 fertilized. At the day of transfer, we transferred in 1, and froze 2. The other two they wanted to give an extra day in the lab to see...and they died off.
We got a BFN in early April...and just had our two precious baby embryos transferred in on the 22nd (FET).

" ICSY is a really cool thing. My DH and i tried this about 3 years ago. Retrieved 4 eggs but none fertilized. It didn't make sense to me because i thought it was a no brainer that we would have embryos since they PUT the sperm in the egg. But apparently there are chemical reactions that must jive for success.

I am actually starting my fertility meds next week and looking at about 2 weeks before egg retrieval. We are going to attemp ivf with icsy again.

Keep me posted...it would be nice to go through this with another couple!! "

" Good luck with the rest of your testing and then future cycle! My DH and I completed our second IVF with ICSI cycle in June and I am currently 12 weeks pg.

As far as how many to transfer, like you said there are a ton of factors. Not to mention personal beliefs and feelings of multiples. All testing we had done showed only male factor so on our first IVF with ICSI cycle we transferred 3, day 3 embies. 2 were perfect quality and the other not to good but it was all we had so we wanted all put back in. Unfortunately that cycle ended in a very early miscarriage.

Our 2nd cycle we again transferred 3, day 3 embies. 2 again were perfect quality and the 3rd was a tiny bit less. We ended up getting pg with twins but at 8 weeks one of the babies died.

" Thanks for all the replies. We had to wait an extra month before starting stim meds because of an ovarian cyst (which went away), but we're back on track now. If everything goes according to plan, we should be transferring embryos in about a month.

We decided on the Attain IVF program (more money upfront, up to 3 fresh and 3 frozen cycles until you take a baby home from the hospital or you get $ back) because we both ended up having issues (mine: severe MFI, hers: one ovary with few follicles).

I've been taking supplements for months now and changed some lifestyle things, all in the hope that I increase the quantity/quality of my swimmers. Even though you only need one sperm for each egg to do ICSI, I want the best ones possible!

I have no idea if all the things I've changed have made any difference (I have not done a repeat SA since taking everything), but we'll see when the time comes for fertilization.

For what it's worth, here's what I've done:
1. minimize alcohol
2. avoid estrogen containing foods (soy, edamame, flax, etc)
3. avoid aspirin
4. exercise
5. nothing hot on the testicles (laptops primarily)
6. Accupuncture
7. supplements: Coenzyme Q10 (100mg/day), Grape seed extract (100mg/day), Multivitiamin, Vitamin C (total of 1000mg/day), Carnitine (both L-carnitine and carnitine), and fish oil.

So we'll see what happens this month--hoping for the best! "
