Need a release.

I’m constantly aroused and have spontaneous orgasms, (read my profile for more info) but its very hard for me to have any release from sexual activity. Because of the injury, I’m always aroused, leak seminal fluid and have spontaneous orgasms. Erections seldom last very long not because of any reason except that I just can’t get excited that much. Oh, I’m aroused, sometimes very much so but I am emotionally exhausted and just can’t get interested in sex yet I yearn for some sort of release.
Masturbation is useless. I’ve used vibrators and that helps some. My wife will use her hands and that helps too but even though I have many orgasms, there is no sense of release. What I really want is oral but it just isn’t practical when I also have urinary incontinence. Even when the incontinence is not an issue, the seminal leakage is a problem. My wife does not like the taste of semen and can’t do much for me. I’m also sure that after a while, there is a bit of urine mixed with the semen. Its not something I can really ask my wife to do. When I’m aroused, I’ll leak a lot of seminal fluid and have a number of orgasms before I can get a release. I’ll usually only have an erection for a short period at the beginning and the end. A very hard erection is a sign that an actual release is imminent. Usually my erections are soft to medium and don’t last long. Anyone have any suggestion on what to try? I've done the rounds with the medical establismment and I'm not very impressed. My chiro (a woman no les) suggested the vibrator and it helps. She also suggested regular physcial exercise. Taking a good walk in the evening does help calm my adrenals and helps a lot.
