I have NO idea what to do about applying for my husbands disability. We have a phone interview next week. I JUST NOW was handed the forms by my husband, who no surprise, forgot to hand them to me. I have to aquire a copy of his birth certificate. 1 problem. HE CANNOT remember where he was born at, or even ever knowing where he was born at. I am relatively certain I remember having the conversation in his prescence with his mom. Shes deceased, his dad has dementia, and his granma can't recall either. Now what? I cannot take a chance of being wrong. Also, he has his work history all mixed up in dates and pays, and remebers working somewhere I KNOW HE NEVER DID! Plus his medical history prior to my meeting him, is a complete mystery. They want 15 years work history. I've only known him for 12.5 years, and neither he nor I can figure those last 2.5 years out. He thinks we've been together 15 years. He keeps showing up for work on off days, and vice-versus even though he looks at the schedule everyday. I am just SO frustrated. How do I answer questions when I simply CANNOT DO IT?!

be honest about it when you speak with disability people...they can assist you with information that you need that you dont have...is you go onto the soc sec website it can answer almost all the questions you have...as far as birth cert...they probly can help you with it...
be sure to do all the paperwork that allows you to speak/do paperwork for him.. as long as you are honest with the clerks they will do what they can to help you..dont make stuff up...just say he doesnt remember if he doesnt... i know you have trouble with finding docs..but you may have to have him evaluated for psychiatric for soc sec disability...they can help you with that too..but if he is having recall issues etc...he should be soundly evaluated...and a psychiatrist is a medical doctor who is familiar with all kinds of memory issues also.. best of luck on the disability process...it will take a long time...just be patient...generally five months after they determine a date of disability they have to make a decision... i applied in may of 08 with a disability date of april 08...last day i worked...and only received funds 12/09 with a decision i think came in oct or nov of 08... if you folks have minor children..you may be eligible for benefits for them...they will ask that... i found that dealing with folks from disabilty to be very patient and very knowledgable...if you have any questions...ask them... good luck to you and hubby.. marie
