5 lbs to go and feeling stuck

I have lost 95 lbs after gastric sleeve surgery and have 5 lbs to go but feel myself slipping - snacking more, not writing things down, increased fat and carb intake. Any words of wisdom from those that have been there? I'm 7 months post-op. Many thanks!
