How to exit a PM's office and start a new

I'm on day 2 of my fentanyl patch and I'm miserable! Upset stomach, groggy, arm pain where the patch is placed, and pain is there worse then ever. They refuse to give me anything for breakthru pain so I have nothing to take as of right now. I called there office today and left a message stating all of the above and never got a call back today. I'm still scratching my head as to why they didn't want to continue me on the original oxy treatment plan. 30mg's is way to much I guess. My chiropractor and boss was perplexed as well he offered to call this PM doctor and tell them how much more productive I was at work and how I wasn't in any pain. Not sure if I will have him do that yet. He gave me the name of another PM doctor in my area and offered to call this doctor and tell him what's been going on. Of course I need a referral from my PCP in order to be seen by this doctor because they make recommendations for treatment plans only. So I called my PCP and left her a message asking her to call me so I could ask her. I just don't feel good about my current PM doc and I'm very frustrated with them. How do I exit from one PM and start another? I have no idea what to do. I'm gonna give my patches back to my current PM doctor before leaving. Any advice on this would be greatly appreciated. Thanks : )

" Hi Kmiller ~

I am a strong proponent of being direct (but polite!) and honest. This may be an obvious question but do you have to see this PM again before you switch to another one? Is it because you have to return the patches? I am not getting the feeling from your post that you are completely certain about leaving this PM, had you a good relationship with this doctor prior to the Oxy being taken? So if the Oxy matter was explained satisfactorily would you stay, or do you feel like you feel there is a lack of trust now?

If you are determined to leave - and - you have to have one final meeting with this doctor, I would just ask why the Oxy was taken from you and why he/she did not allow you anything for breakthru pain? I am not sure you will like or even get an explanation of that situation but I sense you likely will not feel better until you ask!

" Hi Hun,
I guess in your situation you have to have a referral, I have Medicare and I do not have to have a referral from my PCP I can just choose my PM, not that I am going to do that anytime soon since I love my Doc, but anyway's..... even when I had BCBS of Fla. I was able to pic my own Doc as long as they were in my book of providers.

Like Crafty said, I am a proponent of being direct also. I fired my original surgeon do to his lack of conscious with my situation and after he messed me up royally! I made an appointment and brought my mother with me to be my witness and I told him exactly what I thought of him (I didn't let him talk, LOL) and then told him that I needed copies of all my medical records ( I paid $1.00 a page) and told him that he was fired and walked out the door!
If you feel that this Doc is doing you no favors and is treating you badly is time to go. Make an appointment, bring someone with you to be your advocate and fired her. Before you do that, make sure that you have an appointment with your next PM Doc so you won't be left without the horse and the mount.
Just be honest! Be yourself and things should work out for you. Good luck and keep us posted.
BTW, what Mg's did she prescribed for you? Was it 25? Remember that we were trying to figure that one out the other day? "

" absolutely be direct, get your records and if you tell them your chiro dr. want them you will not have to pay for them. Let him know you are not happy with your treatment with him and you feel you need a more concerend doc. who wants to do a pain plan that actually helps you and will return your calls and answer your concerns. It would not hurt to have a witness with you. "

" KMill, I have to agree with the other peeps. Just tell them (if they asK) which they might not because it probably happens more than any of us realize. But just tell them, get your r4eferral from your PCP as all PM Drs. now require a referral. I think you are stressing too much over this, but I do know the feeling and I stress over the BS too. But always remember its like everything else in life, its NOT always a best scenario with your current PM. If you don't feel comfortable with her care, tell them and find another one. Good Luck Sweetie, I go to a new one on Monday, I don't stress as much as I used to. If I don't get the care I need (been a CP patient for 14 yrs) then I have NO problem going to another Doctor. I have come to realize that they aren't all that they often think they are. Lots of Luck and Hugs, Deb "

" PS,,, I had one Dr. throw the patch at me too... WTF ??? I told my NEXT PM it was like a weird anti-depressant medication I had previously taken back in the day. Back when I was still being treated like a lab rat. I know now what I have tried and what works for me and what doesn't Hugs, Deb "

" I would tell the pain doctor how she is and feel that is not in the best interest for your health and your stress,
Let her know how she makes you nicely.
I am very up front to my pain doctor, they have just changed it over to this idiot who sees me a person who will ask the hard questions.
I have told him i don't your method or you and you have to do what is best for me.He didn't like it but said it took guts to say those things. "

" I travelled to my pm, he was about an hour away. he then moved further away so I thought I'd get another PM. What I did was make an appointment with the new PM to see what he was like. I knew as long as I didn't accept any meds I would be OK. I went and had a new patient appointment and really liked him so when he gave me scripts I took them.

I then went right home and called my original pain doc and told them with the move it was getting hard for me and I was going with a doc closer to me. I just spoke with the receptionist, no problem.

Well after about a year, my new PM stopped taking my insurance so I decided to go back to my original PM. So I made an appointment with my old PM and then waited until my appointment where he took me back. I then came home and called the other PM and told the receptionist that since they no longer took my insurance I was going back to my other doc.

I had no problems and as long as you are not getting scripts from both docs you will be OK.

" Thanks everyone for your help. My insurance does not require a referral but some of the PM offices around the Seattle area require referrals. I got a call from my PM stating they were gonna try MS contin then after I hung up I realized that has morphine in it and I can't take morhpine it makes me horribly sick. So I called them back and the nurse said she'd talk to the doc and find something else. I've only been to this PM twice now. My issue is they took me off something that was working to try something else and I couldn't figure out why. All the doc said was were not gonna prescribe anymore oxy. And that was it. They told me I could take the patch off this morning so I did and now I'm sitting here waiting for a call back. It's 3:15 and they close at 4:30pm. Plus there not open tommorow. I think they are highly incompetent and feel there office is disorganized. I just have a bad feeling about this office. Unofrtunately there are NO PM's in Seattle. This is the hardest thing I've gone thru. I'm gonna have to go thru 4th of July weekend in horrific pain. : (

" I went to my last appointment with a bad one and just never went back. When I arrived in the new ones office, I had all my bottles and records with me. We talked honestly about what works for me and what does not.

This is so hard because if I tell you more, it will give the drug addicts exactly what they need to get more illegally.

Good luck

" I barely made it to the office yesterday and gave them the one oxy left and the fentanyl patches back. They gave me a script for Methadone. Wrote it for a fictional doseage luckily Costco just gave me the 5mg tabs. I took as instructed last night and was horribly sick this morning. I feel like I tied one on the night before with alcohol. So now I have NO pain meds to get me thru the weekend. I'm hurtin bad too. This is the hardest thing I've ever gone thru. Thanks for all your advice!! : ) "
