My mom was diagnosed with CLL in early May of this year. She is at Stage 1 "watchful waiting". She continues to smoke and that scares me. Can smoking increase her chances of CLL transforming into Richters or move her up a stage?

Probably not but it sure won't help her and could lead to lung cancer.You are right to be concerned.
Also most chemo can wreck havoc on the lungs so best to keep them in good shape for down the road.
