I would lean towards an argument is "just" words, but a fight has a physical element? (With some overlap between, and not meaning to imply that words cannot wound.)

How about a disagreement?
Disagreement --> argument --> fight?

Maybe something to do with:

Though then it might just be a discussion rather than an argument.
Discussions are healthy. I don't know whether I can give an actual level/gauge/guide for when things become unhealthy other than the "obvious" like physicality. Shrug. I'm not smart enough to define my thoughts. :D

Yep--my gut lets me know... "gut-clenching" is a very apt term.
