Hospice Transitions Program

Don't know if any of you have heard about this but I found out through a neighbor. Many locations including the county where I live has a "Transitions" program. This is how they describe themselves: "Transitions is a unique end-of-life care program that enables hospices to provide services to people with life limiting illnesses, even while they are still receiving curative care." It is volunteer driven. Alzheimer's patients usually qualify. It's free.
I am now going to get free respite, no income or asset guidelines for this. The manager came to the house and interviewed me and my mom. It will take a few weeks to get a volunteer set up for us (trained by hospice, fingerprinted and background checked). Then the volunteer and I work out a schedule and I will have 1 to 2 hours a week where I can actually leave the house without my mom. The manager will also check on us every couple weeks by phone. They will stay with us until or when mom eventually moves into needing hospice care. There are multiple services they provide all free. I had never heard of this. Thought you'd all like to know. Yes, we have this in my area. We were under the transition program for 5 months before going to full Hospice Care at home.
Our transition provided a volunteer for respite, a nurse once a week to check vitals and general health, and a shower aid. Transition is a program before hospice. In our area it is funded through United Way. Full hospice is paid by medicare for those on medicare.

I have yet to even contact the Alzheimers association!! I guess I am missing the boat and should reach out. I just kept thinking I had to be at the END before asking for help.

Sher - The transition program is through your local hospice.
Ask your Dr. if he thinks you are ready. Hospice does an evaluation to determine if you are "there" yet.

I actually have them coming in on Monday to do an evaluation. They have to get a statement from the doctor saying that he thinks that it is necessary. There are many good reasons to inquire about this. One, and the one I think is the most important is that if my spouse has a problem and needs medical attention, he will be turned over to hospice care and will not be treated. I do not want him treated for anything, even a simple cold. I want him to be made comfortable and that is what hospice will provide for him.

I don't think his dr would say we are ready as we just saw him and to be honest my husband presented himself fairly well. Wouldn't you know. Anyway his Dr wants us to start PT as his muscles are tightening up and becoming stiff almost hard and he needs fall prevention.
He said his is a little more than most due to the "Parkinsonism." Dementia w/ Lewy Body is a cross between Alz and Parkinsons, carries bth symtoms.

My husband is between a 5 and 6, but the hospice I am hoping to get him on right now is an intermediate care. They can provide help as I need it, even giving me time away now and then. I am not sure that the Dr, will approve it right now, but it seems worth a try.
